More #include cleanup per POSIX:
 - <sys/types.h>: don't pull in <sys/select.h>
 - <sys/select.h>: don't pull in <sys/time.h>, but rather declare timeval
   and timespec locally
 - <sys/time.h>: *do* always pull in <sys/select.h>
 - <sys/socket.h>: declare timeval if __BSD_VISIBLE for struct splice
