sync to 1.9.14: rewrite escape sequence handling:
- new function a2roffdeco
- font modes (\f) only affect the current stack point
- implement scaling (\s)
- implement space suppression (\c)
- implement non-breaking space (\~) in -Tascii
- many manual improvements
